OHIO — Two people were killed in a four-vehicle accident on State Route 115 near US 30 on Wednesday morning, September 10, 2025. The Ohio State Highway Patrol reported that the accident occurred around 8:33 a.m.

Authorities say Sean Triplett, 40, of Florence, Kentucky, was driving a 2009 Toyota Avalon northbound when he crossed the center line and collided with Morgan Krouse, 24, of Kalida, who was driving a southbound 2025 Chevrolet Equinox.

Triplett then drove into the southbound lane and collided with another Chevrolet Equinox, a 2022 model driven by Joann Kahle, 60, of Fort Jennings, which forced her vehicle off the road.

The initial impact pushed Krouse’s vehicle into a northbound 2008 Chevrolet Tahoe driven by Thora George, 30, of Lima. Both Triplett and Kahle were pronounced dead on the scene. Authorities noted that Triplett was not wearing a seat belt. Krouse and George were treated on the scene for possible injuries and released.

The Ohio State Highway Patrol investigated the crash, with assistance from the Allen County Coroner’s Office, American Township Fire and EMS, American Township Police, Lima Fire Department, and several towing and recovery companies.
